Women's CHOICES Transitions

CCA serves women incarcerated at Onondaga County Correctional Facility who are nearing release and who may be at risk of, or infected by, HIV/AIDS. Services include pre-release and community support groups, discharge planning, and jail and community-based case management.

There are several components to the program. The Empowerment Group, Discharge Planning and Jail-Based Case Management are conducted with the incarcerated women. Prevention Case Management, Community Service Plan Development and the Community Support Group are services provided following release.

The program assists women to successfully make the difficult transition from jail back to their communities and to build new lives based on healthier behaviors.
